Required Mortuary Forms Listing

We have made available to you the necessary forms required by the funeral home and crematory along with legal forms required by state, county and city officials for the release, and burial or cremation of the decedent's remains. Please print the required forms, fill in as much information as possible, sign, print name and date where required. You would then bring these forms with you during the arrangement conference.

Start by filling out a release form if necessary (or instructed to do so by a funeral director).

Then skip below to the burial forms section or cremation forms section (depending on the arrangements to be made).

 

General Release Form

This is a General Release Form required by some facilities (hospital, nursing home, etc) to release the remains of the deceased to a funeral home or crematory.

 

Riverside County Coroner Release Form

This is the Riverside County form required to have the sheriff-coroner release the remains of the deceased to a funeral home or crematory.

 

San Bernardino Coroner Release Form

This is the San Bernardino County form required to have the sheriff-coroner release the remains of the deceased to a funeral home or crematory.

 

San Diego Coroner Release Form

This is the San Diego County form required to have the medical examiner release the remains of the deceased to a funeral home or crematory.

 

Los Angeles Coroner Release Form

This is the Los Angeles County form required to have the medical examiner release the remains of the deceased to a funeral home or crematory.

 

Orange County Medical Examiner

This is the Orange County Medical Examiner form required to have the medical examiner release the remains of the deceased to a funeral home or crematory.

 

Ventura County Medical Examiner - Coroner Release Form

This is a Ventura County form required to have the medical examiner-coroner to release the remains of the deceased to a funeral home or crematory.

 

Burial Forms

Authorization For Disposition With or Without Embalming
This form is required to give authorization to embalm or not embalm the body of the deceased.

Disclosure of_Preneed_Funeral_Agreement
This form is required to acknowledge that any pre-need instructions were presented. Pre-Need documents are written instructions regarding goods or services or both goods and services for final disposition of human remains, when the goods or services are not provided until the time of death

 

Cremation Forms

Authorization For Disposition With or Without Embalming
This form is required to give authorization to embalm or not embalm the body of the deceased.

Declaration for Disposition of Cremated Remains
This form is required to declare how the remains of the deceased are cared for.

Southern California Cremation Authorization

This form is required by the crematory in order to perform the required cremation and release the remains after cremation to designated people.
